It is the official documentation AULA requires for internship enrollment. Students are expected to fill out this form, attach a proposal that includes a list of internship tasks and internship learning objectives, and gather the signatures of site supervisor, AULA academic advisor, internship evaluator and internship coordinator before submission to Registrar's office. The Internship Program Coordinator helps students work on their proposals between weeks 7-11 of the quarter before internship begins.
