Financial Aid Request Form 2014-2015
The form can be found under the Financial section when you login to MyAntioch. We use the information you submit to determine your aid requirements for each academic year from July 1st through June 30th. We also have a paper version that you can mail, fax, or scan and email back to us here.
If you have been notified that your FAFSA was selected for a process called “verification,” please submit the form requested below for the 2014-15 award year:
In addition, you may be required to submit a copy of your IRS Tax Return Transcript (and one from your parent(s) if you are a dependent student). Transcripts can be ordered by calling 800-908-9946 or online at the IRS website.
NEW STUDENT GRANT APPLICATION. If you are new to Antioch University or enrolling in a new program, you can apply for this grant by reading the instructions and submitting the application. Keep in mind: there are deadlines, so be sure to read the instructions carefully.
If you are interested in being considered for a Federal Perkins Loan, please contact our offices to determine your eligibility and availability of funds. If awarded, you will be asked to complete the online Entrance Counseling and Master Promissory Note.
